Atypical presentation of Enterobius vermicularis in adults

Introduction: Enterobius vermicularis is a nematode that exclusively parasitizes humans. Adult and gravid females of the parasite migrate, generally at night, to the anus to deposit their eggs, and then return to the colon, where they reside. During this migration, some females may find adjacent orifices such as the female genital tract, colonizing it. Objective: To demonstrate the possibility of transmission of the parasite to age groups not considered susceptible. Methods: We present the case of a 23-year-old woman who presented with colicky abdominal pain, loose and occasionally diarrheal stools, and irritation and itching of the anus, pubic and vulvar region, as well as mild vaginal discharge. Results: A gynecological clinical examination with cytology was performed, which revealed abnormalities consistent with bacterial vaginitis. Similarly, microscopic examination of stool and vulvar secretions was performed, revealing developmental stages compatible with Enterobius vermicularis in both. The patient was treated with a single oral dose of 400 mg of albendazole, repeated after 14 days, resulting in resolution of the infection. Conclusions: Although this is not the first case of enterobiasis in adults, this research underscores the importance of disseminating information to reduce the risks associated with the infection.
