Relationship between depressive symptoms and treatment adherence in patients with type 2 diabetes mellitus

Depression and type 2 diabetes mellitus (T2DM) often coexist, negatively affecting self-care and metabolic control. The study aimed to determine the relationship between depressive symptoms and treatment adherence in patients with T2DM treated at the Punta del Este Health Center during 2025. An analytical, cross-sectional, and prospective study was conducted in 120 patients diagnosed with T2DM. Depression was assessed using the Zung Self-Rating Depression Scale, and adherence was evaluated with the Morisky-Green-Levine Test. A total of 61.7% of patients were non-adherent to treatment, 40,8% presented mild depression, and 2.5% moderate depression. Multivariate analysis showed that mild depression was significantly associated with lower adherence (aPR: 0.43; 95% CI: 0.24–0.77; p=0.004), while moderate depression showed no significant association (p=0.450). It was concluded that even mild depressive symptoms negatively influenced therapeutic adherence, highlighting the need to integrate mental health into the comprehensive management of T2DM at the primary level of care.
